What is our post-surgical service?

We believe that the best place to heal is at home. Our Post-Surgical or Hospital Care can help you at this challenging time.

Discharged patients from the hospital go through a complex process which is fraught with challenges, resulting in unplanned readmissions. Preventing avoidable readmissions has the potential to profoundly improve both the quality of life for patients and the financial well-being of healthcare systems.

We strive to provide experienced caregivers who will do:

  • Post-discharge monitoring

  • Medication reminders

  • Compliance with doctor’s discharge orders


Why is this so important?

Hospice care services both the patient and their family, relieving the stress of illness and allowing the focus to shift toward closure.

•        A lack of knowledge

•        Early discharge from hospitals

•        Poor medical oversight

•        Community’s inability to afford professional help (home health nurse visits can cost up to $225.00) 

•        Refusal to be in nursing homes because of a traumatic past experience involving poor quality of care or acquired infections, such as VRE, C-DIFF, MRSA, due to inadequate funding in these facilities

•        Fear of  losing one’s Home, their haven, is a strong element in patients’ sense of security.  A certain amount of control is lost when a person gives up their home. This “control” provides the basis of our feelings of dignity, quality of life, and independence.

•        Cognitive disorders such as dementia and Alzheimer’s

Why us?

Healthy At Home Caregivers has a 1% rate of readmission. This 1% is due mostly to advanced and unavoidable disease processes.

Healthy at Home caregivers is owned and operated by Registered Nurses, who always provide a complimentary initial assessment while coordinating care to avoid interruptions in your recovery. The registered Nurses will stand by your side to guide, protect, and support you throughout your care.
